I've just noticed that the toolbar doesn't appear when editing your first post in a topic. (it only appears on editing following posts).
Using Firefox 42 with Greasemonkey 3.6 (or Nightly 45 with GM 3.7 b1).
There are no entries in Browser Console while clicking Options|Edit for such a post.
